Contest Rules and Regulations
- To be eligible, you must be a member of NeuroRehabResource
- Odds of winning depend on the number of eligible entries received
- Three winners will be randomly selected from surveys submitted through May 7, 2010
- Only one entry per person
- Winners will be notified by e-mail May 10, 2010
- Void where prohibited by law
- CogniMed Inc. reserves the right to refuse any submission for any reason
- Prizes are nontransferable and not redeemable for cash
- Employees of CogniMed Inc. are not eligible
- All federal, state, and local laws and regulations apply
- Participating entrants agree that CogniMed Inc. and its subsidiaries, officers, directors, and employees shall not be responsible for lost, misdirected, illegible, or late entries
- By registering, contestants accept these rules and agree to be bound by the results of the prize drawings. All decisions made by CogniMed Inc. are final
- The 2009 book Botulinum Toxin: Therapeutic Clinical Practice and Science by Joseph Jankovic, MD, et al is subject to availability. CogniMed Inc. reserves the right to substitute the prize for one of equal or higher value
- Participating entrants agree that CogniMed Inc. can post their name on the NeuroRehabResource.org website if they win the contest
